The consent form
Set up the consent form participants sign before they start, from standard wording, with your own tick boxes, versions and publishing.
Participants read and sign the consent form before they start. You cannot invite anyone or take a study live until the form is published. This article shows how to set it up.
Start the form
- Open the study and click Consent. Owners and researchers see this tab.
- Click Start with standard wording.
Zolensa fills in a title, an information text and four tick boxes in each study language. Check this standard wording with your data protection officer before you publish.
Choose the tick boxes
Under Tick boxes, choose which boxes are Shown and which are Required, then click Save tick boxes. Participants must tick every required box to join. Optional boxes are recorded as yes or no.
The standard tick boxes are:
- Take part in the study: always shown and always required.
- Be recorded on video (face, voice, home): required at the start.
- Clips may be shown to the client: optional at the start.
- May be contacted for future research: optional at the start.
Add your own tick boxes
- Type the text under Add your own tick box, in the language you have open.
- Tick Required if participants must agree.
- Click Add tick box.
You can add up to 3 of your own. They are always shown. Add their wording in the study's other languages too. To delete one, click Remove next to it.
Edit the wording
- Click a language at the top. Each language shows a tick, or how many parts are missing.
- Under Wording in, edit the Title, the Information text and the text of each tick box.
- Click Save followed by the language name, for example Save English wording.
In the information text, leave a blank line between paragraphs. These words fill in automatically: {organisation} becomes your workspace name, {study} the study name, and {days} the number of days data is kept.
For languages other than the main one, Translate from English with AI (with your main language) fills in the boxes. Nothing is saved until you click Save. People sign this text, so have a native speaker check it.
Reset to standard wording replaces the title, the information text and the standard tick boxes in that language. Your own tick boxes are kept.
What participants see shows a preview in the language you have open, with the signature box and the I agree button. Participants sign by typing their full name.
Publish
- Make sure every study language has a title, an information text and wording for every tick box shown. The Publish box lists anything missing.
- Click Publish version 1.
New participants now see this version. A published version cannot be changed, so participants always know what they agreed to. Discard draft deletes a draft you do not need.
Change a published form
- Click Change wording (start v2), or the next version number. This starts the next version as a copy of the published one.
- Make your changes.
- Publish the new version.
Until you publish, participants keep seeing the old version. People who have already signed are not asked to sign again.
Published versions lists every version, when it was published and by whom. On each participant's page you can see which version they signed, in which language and when.
Note: If you add a language to the study after publishing, start a new version and add the wording in that language. Until then, people who read that language see the form in English or the main language.
